Research Summary
The research interests of my laboratory and clinical staff are to understand the mechanisms of autoimmune disease, in particular, autoimmune diabetes, and develop novel approaches to treatment of this as well as other autoimmune diseases.

Research Activities
Our studies involve two clinical programs and basic studies that support this work. We are studying the effects of a non-FcR binding anti-CD3 antibody on human Type 1 diabetes. In a Phase I/II trial, we have shown that treatment with this antibody can reduce the rate of loss of insulin secretion in patients with new onset disease. Further clinical studies are in progress to determine whether the effect of the drug can be maintained for a prolonged period and to address whether there is evidence for beta cell regeneration in patients who are treated with the drug after induction of tolerance to autoimmunity. Parallel studies are being carried out in an animal model of Type 1 diabetes to determine whether beta cell regeneration occurs in the remission of diabetes that is induced with anti-CD3 antibody.
Our studies have suggested that treatment with anti-CD3 antibody may induce regulatory cells and/or phenomenon. Our laboratory is focused on identifying cells and/or mediators of inhibition of autoimmunity and understanding how they function.
In addition to inducing tolerance to autoimmunity with anti-CD3 antibody, clinical and basic studies are ongoing to restore lost beta cells with islet transplantation. Our first clinical transplants are being done in patients with pre-existing renal transplants. In addition, we are studying ways of modifying the islet allografts to be transplanted in ways that will prevent recurrent autoimmunity and allorejection. As an example, we are studying the effects of a soluble inhibitor of RAGE (receptor for advanced glycation endproducts) on this process.
